Every day my account is suspended. I have removed the items you asked me to. Why is it suspended AGAIN....so frustrating

I did check your form "Subcontractor Registration Form" and found that you still have form fields asking for "National Insurance number", "Full UK Drivers Licence", "UTR Number" and "CSCS card number" that seems to be causing this issue.
I have deleted the suspected form fields and activated your account again.
