Using Spreadsheets Widget to Recreate a Form

Hello,

I am using Spreadsheets Widget to recreate the attached form. I am able to inject CSS into the rows and add labels to them. However, when I preview the form, none of my injected CSS shows up and none of the labels display. I just need to be able to have my applicants to complete this simple budget form that will calculate their projected income and expenses and then projected profit/loss... the calculations are simple addition and subtraction, but I am having a terrible time getting this to work.
