I think I deleted my form

    I deleted an older website and think my form went with it. Can you check and see?

    As long as you did not delete the form on your JotForm account, it should still be there or you can check the Trash folder and restore the deleted forms.

    I checked your email address (dr.matthewsbarnes12@gmail.com) and it is registered to the account username Mattman12.
