How to include Text fields in PDF?

This form does not pickup the additional ADD FIELDS i add to the customised pdf.

I want to add the privacy notice and student declaration to the beginning of the pdf template, but it just wont pick it up.

if you refer to the screen shot attached, i am showing where i want it, which i seem to be able to add, but then when i preview it , it is not showing the new fields.

Please try going to Submissions page and clicking this option:

That will show Text field by default.
