I am using Adobe Muse and the form is not visible. I have updated the form a few time to get it to show. This is critical!!! My client is at an event and they are sending people to this site.

Thank you for you additional info, I viewed your form in mobile device and replicated the issue.
I checked your form code, and you have embedded your form with an IFrame code.
Kindly remove re-embed your form using the IFrame code without the java script.
