FORM NOT VISIBLE!

I am using Adobe Muse and the form is not visible. I have updated the form a few time to get it to show. This is critical!!! My client is at an event and they are sending people to this site. 

I checked your form, and I can't seem to replicate the issue. I was able to see the form in your website.

I checked your form code, and you have embedded your form with an IFrame code.

Kindly remove re-embed your form using the IFrame code without the java script.
