Card form: Not all product options appear

    I'm unable to figure out what is wrong.  I have entered 4 product options, but only 2 appear on the form.  I've looked at all settings and they appear to be correct.  What is wrong?  Please help!

    Yes everything is correct. And its supposed to work like that. Only two of your questions are shown on the card at once.
