I am sorry for the trouble caused to you. I did check your form and did not find any issue with that. You have integrated your form correctly with Square account. I could not test your form since it requires active credit card to submit form.
I would suggest you to please test your form again and get back to us if the issue persists.I would also suggest you to please enable "Test Mode" of your payment tool so that we can test your form. Please check the screenshot below on how to enable "Test Mode":
