I can't fix the format in the template

I cannot seem to fix the formatting.  The question fields are very narrow causing what should be a one line question to wrap to 4-5 lines.  How can I fix that?

In order to fix this, you need to add Custom CSS. Custom CSS can be added to your form as follow,

1. You need to access your form "Form Designer" option

2. Then you need to click "CSS" tab.

3. Add the following CSS code block into that area.

#label_5, #label_7, #label_11 {

    width: 100%;

}

Thats all! Your form going to be displayed as you wish!
