    Fine Folks at Jotform,

    I've had an account with you for over 6 years and I haven't had any issues. This is my second request to notify me why my account was suspended or if there is any remedy I can make to restore it.

    I checked your account and your forms and I saw that there was a field that asks to upload driver's license. The information that belongs to form users such as password, SSN, credit card number, CVV, driver's licenses are considered as sensitive personal information. Please note that asking sensitive personal information violates our Terms of Use.  That's why I deleted this field from the form and reactivated account.

    Please note that, you are required to use our Terms and Conditions or Signature widget on your forms in order to ask driver's license.

    Hi Tina,
    Thank you so much for letting me know what caused it. The form is a
    verification for candidates running for local office to verify that they
    live in the District they are running for. We'll remove driver's license as
    an option of verification.
