Widgets: Other option request in the orderable list widget

I need this kind of question type.

Among the ff. qualities, please pick out you top 3 qualities (top 1, top 2, top 3).

Others please specify.

I have tried two methods for this. One is limiting multiple choice to 3 choices but this does not allow me to get the rank 1-3. I also tried orderable list, but this does not allow people to input anything in the "Others" field. Any help?
