How to capture the survey field on visual report?

When I tried to run a visual report builder, I only see 2 questions on the report. I tried using the filter, but no luck. I like to use the visual builder report because it provides graphical analysis. I do not like grid or table report because those are just spreadsheet. Can you please help me with the report to capture the survey data? Thanks.

You can capture the survey data by clicking on the settings or the gear icon then selecting the survey field.
