Importing Excel to JotForm

    I'm trying to upload an Excel spreadsheet to JotForm using the Import Data app.  The .xls template format isn't showing all of the fields in my spreadsheet.  I'm not sure if that is the issue and why the submission is showing as blank in Jotforms. 

    I've also tried several different formats of date of birth (the one suggested is yyyy-mm-dd h:mm but I'm not sure how to get the phone number format as suggested.  I've deleted the phone number to avoid that being the issue and it still doesn't work.  The results are showing successfully upload but it is creating blank submissions and multiple submissions.

    Sorry for the inconvenience caused to you.

    As I checked your account, there are several forms on your account. Could you please provide us the form ID?

    If I understood your concern correctly, the date of birth field and phone number field are missing on imported submissions. If the missing fields are more than the fields I have mentioned above, it would be helpful if you provide us all of the field names which are shown blank when your file has been imported.

    It is on the CareHub Intake Form.

    Actually all the fields are blank on the imported submissions. 

    When I run the import, it says it was successful but it creates empty records; all it shows is the submission date.

    I mentioned date of birth and phone number because the requested format on the import template (yyyy-mm-dd 0:00 and 01234,2345678) isn't the standard format we use (mm-dd-yyyy and 517-234-2345) for these fields so I thought maybe that was affecting the import.  

    Also, our form has column headings going to AV on Excel, but on the import template it only shows fields to AF. 

    I've tried it in Chrome and Internet Explorer.

    I have checked your form and it seems that you have already imported the submissions to the form. I see that there are 491 submissions in the form currently. Were you able to import the submissions? If you still need any further assistance, please let us know. We will be happy to help.
