Why prepopulated URL is not working?

    There is an issue with your link: 

    https://form.jotformeu.com/80640743203347?patient_first_name=James&patient_surname=Dodd&patient_email=doddjb@yahoo.com.au&patient_phone_number=?&drs_name=Jonathan%20Field&drs_email=jon@docto.com

    You should have a value there:

    https://form.jotformeu.com/80640743203347?patient_first_name=James&patient_surname=Dodd&patient_email=doddjb@yahoo.com.au&patient_phone_number=000000&drs_name=Jonathan%20Field&drs_email=jon@docto.com

    So it could work.

    I see! So if one parameter is blank the others don't work?

    No that is not an issue here. It is not required to populate all form field, the issue with your form's pre-population URL is the use of "?" which was causing this issue. Your URL with parameters can only have one "?".
