Need Account Un-Suspended

I was made aware of why my account was suspended. I need to remove fields that request “login” information from users as this is agains Jotform’s terms. 

However I need my account un-suspended in order to gain access and remove those fields. 

It looks like this is a duplicate thread and it has already been answered by one of my colleagues here.
