How do I add my form to ebay?

I'm trying to add jotform to my ebay listings https://form.jotform.com/80727880484164

ebay does take javascript

I have tried to edit the one I have with no success

After countless hours I got it, in case anybody else needs it here's how I did it

I tested it, it works
