Forms disappeared and I can't log in

I built a form which I have been using but it's disappeared???

Why has it gone? I also can't seem to log in

I checked your account,  LucyBrownTATTOOS it is active and has two forms int it.

May we know the FormID or name of the Form missing?

You can try to get a new password, by clicking forgot password.
