Shipping Quotation form with order form...

Hello,

 I am trying to create a shipping quotation form that allows a client to enter their pickup and delivery addresses which then gives them a quote based on a flat pickup fee plus a per mile fee. I have a basic form more or less working with the driving distance and form calculations widget... but it isnt quite doing what I would like. I would like an "Order Pickup" button to show after they get the quote.... which will then take them to an order form to enter all the pertinent details with stripe payment gateway. 

  I am sure your forms are capable of this and if they are I would be happy to upgrade my account. 

It would also be nice if the driving distance widget would display a map?
