Account Suspended?

Hi there,

I was creating a few forms last night for a mockup website I'm doing for grad school & my account says "autosuspended"... Wondering what's going on?

If you are referring to the account username "jpechin", where the following form is hosted: https://www.jotform.com/80918102387155, the reason why the account is suspended is due to that form that is requesting login credentials, that is something that goes against our terms of use: https://www.jotform.com/help/8-Terms-of-Use 

"Phishing. You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."

That form must be deleted if you want to have the account reactivated.
