I have two account profiles and I want to delete one...but I've guessed at all the username and password combos I can think that I would have used and I'm not able to log in on the account I want to delete.
It is the account that is tied to this email address. firstname.lastname@example.org
How can I get my username and password for this account so I can log in?

I see that the email address is associated with the account jhoughtalen. However the account is suspended due to the form http://www.jotform.us/form/80806904762158 which is collecting SSN on the form. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.
In order to activate your account, it is required to remove the guilty fields on the form. Please confirm so that we can delete the guilty fields and activate your account.

Ok. Do you have already have another account? If you need to register a new account using the same email address, we can actually delete the form/guilty fields from the form and activate your account so that you can continue using the account. If you want to change the account username, we can change it too.
For now, I have now deleted the guilty fields on the form and activated your account. If you still want to delete the account, you may change the account email address from the account settings and let us know here so that we can proceed with deleting your account.
