Thank You Page: Reload form in the same tab

How do I add a link to my form on the submission page to make another submission. I do not want it to open in a new tab or page, I want it to replace the existing tab or page with a fresh form.

You will need to correct the link in a source code a bit, just replace _blank with _self to load the form in the same tab.
