Form:Need to add another field at the bottom of the form.

At the bottom of the form I need to add another field for Agency/Company. In this order:

Name

Agency/Company

Email address

Can you walk me through that?

You can use Short Text Entry to ask for Agency/Company.

Then you can just drag and drop the Agency/Company field below the Name.
