Hello, I would like to know if I can create an exam in jotform and when completing the score I can download a pdf?

Thanks for reaching out to Jotform Support. Yes, you can create an exam or a quiz form in Jotform and attach a generate PDF to the Notification Email so you can download it every time someone completes the exam. Jotform has a free Quiz Maker, which gives you multiple Quiz Templates that you can choose from. Let me walk you through setting that up:
- Go to Quiz Maker, and click on the Create Your Quiz button.
- Select the template that you want by clicking on Use Template.
- Edit the questions, as well as the options in Form Builder.
If you prefer to start from scratch instead of selecting a template, just click on the 'Back' button in the upper-left corner of the page, and select 'Start from Scratch'. You can then select between Classic Form and Card Form for the Form Layout:
Next, for you to get a generate PDF after each form submission, you can attach a PDF to your Notification Emails, so you can download the PDF from the email notification. Let's do it:
- In Form Builder, click on Settings in the orange navigation bar at the top part of the page, and go to Emails.
- Hover your mouse over Notification 1, and click on the pencil icon.
- Go to the Advanced tab, and toggle Attach PDF. Tick the PDF Document below.
- Click Save.
If you want to modify the PDF format, you can click on the small arrow on the far right of the PDF Document before clicking on Save. Check out the screencast below:

It's excellent. But I would like that when you finish the exam and achieve a score, you can download the passing certificate, otherwise, if the score is not achieved, you must repeat the exam.

Hello ITHEMI,
Thanks for your patience and understanding, we appreciate it. I've created an example of a quiz form to help you get your ideal workflow. In this form, if a form responder fails the exam, they have the option to retake it or not. Let me walk you through it:
- I added some basic elements like Full Name, Email, and Single Choice for the quiz sample.
- I also add a Page Break element that will divide the first page into the next pages.
- On Page 2, I added a Section Collapse element that will display if the form respondents fail.
- On the same page, I included the Paragraph element that will show once you click on the Section Collapse once they fail the exam.
- I also added the Form Calculation widget, which will calculate the score percentage.
- At the bottom of page 2, I input the Single Choice to ask if the form respondents would like to retake the quiz or not.
- Lastly, I added another Page Break element.
- On Page 3, I added Section Collapse, Paragraph, Form Calculation, and Page Break. This section is if the form respondents pass the exam.
- On the last page of the form, I added the Paragraph and the Submit button. This page is a message to thank the form responders after taking the exams.
- Next, let's enable the Calculation Values of each quiz, so the score will appear. The average percentage will depend on you.
- Do the same steps with the other Single Choice quizzes.
- On both Form Calculation Widgets added on pages 2 and 3, this is the necessary calculation you need to get the percentage result of the exam.
After adding all necessary elements to the form, we'll add the two Autoresponder Emails. The reason is that if the form respondents pass or fail the exam, there are separate emails to be sent to them as confirmation. It's easy, let's do it:
- In the Form Builder, click on Settings in the orange navigation bar at the top of the page.
- Select Emails in the left corner.
- Click on Add Email and choose Autoresponder Email.
- Here are the 2 Autoresponder Emails created.
- To create the Autoresponder for passing the exam, here's how you attach the certificate. Change the Subject Email and click on Insert Image. Choose your certification and click on Save once done.
- Lastly, here's how I made for the Second Autoresponder once the form respondents failed.
Whether your form user passes or fails the exam depends on which context you need to enter for the autoresponder emails. Lastly, We'll set up the important Conditional Logic needed to get the workarounds you need into your form exams. I'll walk you through setting that up:
- Go to Settings at the top.
- Select Conditions in the left corner.
- Choose Show/Hide Field condition.
- This is the First Show/Hide Field condition to show the Page Break 1.
- Here's the Second Show/Hide Field condition if the form user passes the exam. The following fields should show once they pass.
- On the Third Show/Hide Field condition, this is the necessary setup once the exam is failed.
- This is the Fourth Show/Hide Field condition if the form responder wants to retake the exam.
- On the Last Show/Hide Field condition, this is the setup you need if the form responder doesn't want to retake the exam.
- Next, we'll add one Skip To/Hide a Page condition. Just follow the steps 1-3 and choose Skip To/Hide a Page.
- This is the Skip To/Hide a Page condition if the form user wants to retake the exam. They will return to the exam page (Page 1) if they would like to retake the quiz.
- Lastly, create the two Change Email Recipient conditions. This is to send a separate email to the form user if they fail or pass the exam.
- This is the First Change Email Recipient condition needed once the form user achieves the 100% score.
- Finally, this is the Second Change Email Recipient condition if the form user fails.
Once everything is set up, you're good to go. Here's the result after:
