    Sorry about that.
    The questionnaire shows fine on my iPhone. Our VP said it was cut off on his?? I will work with him.

    Upon checking your form in different mobiles, I did not find any irregularities. However, checking on something being "cut-off", could your VP mean the background image?

    If yes, please note that the actual image has white space above it.

    If you would like to remove the white space, please edit the image on your preferred image editor and upload it back by clicking Form Designer > Image > Custom.
