Terms of agreement form

My application will not advance to the next 2 pages past the terms page. I have two more pages that need to also be seen and used by applicant. Please assist

On testing the form it seems to be working fine.

Note that the form will only advance to the next screens if the user agrees to the terms of the agreement.
