Form: Survey form is loading slow

Surveys form on site is super slow and cannot even get it started sometimes by clicking the next button. How can form be sped up to not load so slow?

I have checked your form and it is loading fast using the FORM URL. However, when I first try to visit your website, it seems slow to load at first but when the browser's cache was saved to my local,it was loading fast.
