Unable to submit form due to missing submit button

    I have created a form, but I was not able to send the link through email.  I tried several times and then I copied the link and sent it to myself. I was able to open the link, but I could not submit the form.  I filled out all the required fields, so that's not the issue.  Now I can't even see the link.

    I checked your form and it is missing a submit button:

    Without a submit button, there is no way for users to complete their submission.  The form is available from the URL you listed and is also available from the following URL if that one is not working for some reason:
