Google sheet integration removed on your form

The google sheet integration for the form "Associate Undertaking" 

https://www.jotform.com/form/81351169928464 

was removed automatically. Can we know the reason for it. Or the IP of the user doing the deletion?

I have checked your JotForm and see that the form is having the Google spreadsheet integration now. It is not possible that any changes can be made automatically to a form without any user intervention. Please check once if any other user who might have the access to your account or forms has made the changes. You may check the account history for the users logged in and the IP address from the history section of your account settings.
