Account Suspended

Dear JotForm,

I am an independent insurance broker that manages my own insurance brokerage website.

I create jot forms so that potential customers can request online quotes from their computer. You disabled those forms which is hindering my opportunity to gain new customers. Please reactivate my account or kindly notify me as to why it cannot be activated,

I have checked your account and it was  reported by our phishing detection system as suspicious of phishing activities, your form " Life Insurannce quote Form" is the one that triggered the suspension.

We understand this might not be the case with your form,  yet we had to act according to the general rule to be in compliance with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately.

To lift the suspension, may know if you have a corporate email address and your corporate website?

I have re-activated your account and forms now. 

I recommend you also add your Terms & Conditions using the Terms & Conditions widget and a signature widget on your Insurance forms to make the forms compliance to Jotform Terms.

You have to comply to this as soon as possible to avoid the forms being suspended again due to violation to JotForm terms.

As my colleague has suggested on the previous reply, please add a Terms & Conditions Widget and a Signature Widget to your form so that the users agree (opt-in) to submit that form.

I will let the reviewers know that you do not have a custom domain email and we will let you know if a custom domain email is mandatory.

Please send the documentation that proves you work as an insurance agent or any documentation that proves your business is legit to support@jotform.com

After this, we will whitelist your account to make sure your account does not get suspended again.
