The Storage $5 per month is it 5 forms per month or 5 forms in total?

As I understood, you are asking if the form creation limit is reset monthly. No, you can only create 5 active forms while on Storage plan. If you would like to create more you need to upgrade your plan or disable/archive your existing forms in order to create a new one.
