How to add file upload field in form?

    I have designed a simple form to check documents collected by our registration team they add expiry dates of the document and indicate if they did not get the form or form N/A.

    What i thought is rather than currently uploading the images  of the documents to google drive(current method) it would make  sense to upload to this form so everything is in one place.

    But i could not add a upload button to the right of each row


    can you help

    I did check your form and found that you are using "Input Table" field. Please note that it is not possible to add file upload field in input table. 

    I would suggest you to please add use regular short text entry field for "Copy Taken", "Copy Not Taken" and "N/A". You can add "File upload" field in form to allow users to upload file. Please check the screenshot below:
