    Hello Joseph,

    It seems that our phishing detector found your form suspicious because of type of credit cards in field labels. Please have in mind that this is not a violation of our Terms of Use. However, for future references, it does become a violation if any kind of sensitive credit card information is requested from your customers in the form of an input field.

    Your account has been now reactivated and you can continue developing the form.

    I got logged in after reactivation, and after about ten minutes it is once again autosuspended.

    Incidently, the labels are payment types as this is going to be a shift reconciliation form, and the fields will hold totals.

    Your account has been suspended due to the credit card fields on your form, as stated by my colleague, it's not allowed to ask for such information through plain text fields, although your form may not be phishing we cannot allow to ask for credit card details. If you need to ask for payments you will need to use any of our currently available payment integrations: https://apps.jotform.com/category/payment_processing 

    Now, in order to reactivate your account I have removed the guilty fields, I also whitelisted your IP address.
