    Hi,

    I have a standard form I'd like to add to various pages on my website.

    When the form is submitted I'd like to know what page it was submitted from (without the user needing to input that) - is that possible?

    For context, it is a form that is going to be placed under a video saying "order clips from this film" ... and I don't want them to have to say what the film is - but I'd like to use the same form across hundreds of videos.

    You can include the parent URL address to your form by using "Get Form Page URL" widget

    From Form Builder:

    1 -  Click "Add Form Element"

    2 - Click "Widgets" from Menu

    3 - Search for "Get Form Page URL" widget and add it


    This widget adds the page URL that forms filled, and you can follow your orders by doing so.

    If you want to show or hide different form elements in different pages:

    1 - Enter Form Builder

    2 - Select settings

    3 - On conditions, click Show / Hide Fields

    4 - Choose If Get Page URL "contains" keyword you choose, do hide/show a field
