Not letting me clone form properly

I am trying to clone a form and it is not working correctly. Attached is a screenshot of what I'm seeing when I go to clone the form. There are two screenshots within the one screenshot since I can only upload one image. One is the form I want to clone and the other is a screenshot of what I see when I go to clone the form.

I need this problem fixed asasp. Please let me know what I can do to begin cloning forms properly.
