Is there a way to make JotForm Fillable Form PDF's cross-application compatible?

    Hello,

    I created a fillable PDF version of the following JotForm - 

    MEDICAL HISTORY FORM - Printable

    Unfortunately I'm unable to upload the attachment, as PDFs cannot be uploaded here (only jpg's and gif's).

    The main issues are:

    Application Compatibility. The form submits perfectly fine when I open the PDF in Adobe Acrobat Reader. However, when I load it in Google Chrome or Microsoft Edge / IE Explorer, nothing happens when I click "Submit." This will be a problem for all of our users who don't have Acrobat Reader installed, or who don't know how to launch it.

     

    Extra Page Breaks. on p. 4 of the PDF, the title ("General Health Issues") appears, with an very long page break before the next question "Please select all conditions that you are experiencing."

    Please let me know how this can be resolved.

    The browsers (Chrome, IE, FF) are not "PDF writer" applications. Those browser technology were not designed for such application. 

    The Adobe Acrobat Reader on the other hand, was designed for that.

    Please understand that this is something that JotForm have no control about. We cannot make the browsers function like the adobe acrobat reader does. 

    Other PDF writer app works also on the PDF form. i.e. the Foxit pdf reader which is also a freeware app. You can suggest this to your users also.
