Submit button is going back to previous page instead of submitting the form

    Hi

    I have created a form called 'Request an appointment' when I have put it on my website http://www.serene-aesthetics.co.uk I have tried to book an appointment and when you put in your details and click confirm, instead of sending me an email and a thankyou page appearing, the screen just flashes back to the first page of the 'Request an appointment screen' how do I fix this so when they confirm appt it gets booked in and they get a thankyou message or a confirmation email

    The final button in your form is a page break instead of a submit button:

    The back button text is set to confirm booking, but is indeed still a back button.  Delete that page break and replace it with the submit button.  The form should then work as expected.
