Track Changes During Data Entry Into Form

    Hi. I am creating a form that has numerous questions (yes/no, text input, etc). I know how to do this. What I would like to do is be able to track any changes made to the answers to my questions. For example, I would like to know if someone answered "Yes" to a question and then noticed that a "Please explain" box appeared so they changed their answer to "No" instead so they didn't have to explain. Another example would be someone that started typing text into an input field and then decided to erase it and re-type something else.
