Some fields of form are not appearing to adding condition

How do i rectify this.

it's not consistent.

i don't want ot have to keep redoing the forms. especially the 16 page documents

I checked your form and found a condition that is related to your missing fields.

If you disable this condition the problem will be solved:

1) Please go to Settings->Conditions and choose 3rd condition:
