Missing field

Hi there! When I go in to edit my form I see a field that has selection boxes for clients to choose from. This field does not have "hide field" selected. When I look at the published form, this field is missing. What's going on?

From what I have checked, that field is using Conditions (Show/Hide).

But the condition assigned to the field have error due to missing or changed field. 

To correct, you can either delete the condition statement or fix it by editing the condition.
