Widget is just displaying loading and not filling the fileds

I created my form and uploaded the excel file but when I publish the form and enter a unique ID from the spreadsheet, the form just says "loading" and does not ever change.

My column headers match the short text entry labels.

Not sure what I am doing incorrectly...

I have checked your account and see there are several forms in it. We request you to get back to us with the form ID/URL and a few unique IDs that can be entered in the widget to check and assist you further.
