I am trying to create a Visual Report Builder report for a survey, but it only does the first 2 questions.

You need to add a new chart to show another question. 

1) Increase the height of the page to add a new chart.

2) Click on the Chart button.

3) Click on the gear icon of the new chart, and change the form field in the "Charge data" option.

So, you need to add a new chart for each form field. Let us know if you have more questions.
