Logo on my form

Getting Close...

1. My logo looks great on the unpublished copy however it goes outside the format on the published version.

2. How do I remove the link (or create one that has been personalized for my business)?  The hyperlink takes you to the sample version.  Almost a big OOOPS!  Thought I was done.

That is correct. I would just like the Logo to be a little smaller so not to run off the edges of what appears to be fanned out pages.

I have not published it yet because I have not resolved the 2nd issue which is the embedded link which takes you to the “sample PDF” I don’t know how to either modify that document with our information.

This is the hyperlink I keep referring to. How do I customize this link to my company logo? It is currently on the first page of the form. Everthing else is perfect. Thank you for your help!!!
