Brower's tab reflects wrong page title

I used a previous form and recreated the titile and details - now when one clicks on the button to register for that event, the previous form title keeps appearing in tab.  Please advise.  It should say Esquire Awards and it says World Aids Day.  

To my understanding, the issue you are having is the title of the page that appears on the web browser's tab. It appears that you have already deleted the form you shared with us and created a new one that is already showing the correct title.
