We created a form but when we attempt to share it there are additional fields being addeded to it a calculation box an then the placement we setup is not the same once we view it from the link. The attached screenshot is how the first page should look and i will paste a picture of the second page below the link address will show you what we get when we go to publish the form.

I'm sorry if I misunderstood, I checked your live preview:

    And PDF version:

Could you please point out where the problem is exactly?

    Thank you!

The PDF version is correct but if you click that link for any of my clients it adds that calculation field moves the signature block and service type and session end time fields.

I checked your live form https://form.jotform.com/90146712892157 but I am confused as to what specifically the issue you described.

Did you perhaps meant the Submit button should be the lowest element/field from top to bottom?

All the fields you mentioned were visible in the live form -- I am not sure if you meant the Session End time and Service type fields should not be visible.

Please clarify and we will check again

Yes the Submit button and Signature block should be at the bottom like the PDF version

In that case, you can simply drag and drop the fields to the bottom of your form.

    Here's a screencast how.
