How to change the page title of the form

How can I change the title of my form as it shows up in the <title> tag in the HTML, which governs what the Title of the form/page is in a browser tab?  I've cloned an old form, and changed the title of the new form in every place I can see, but the old form's Title still remains in the <title> tag and on the tab of my browser.
