Is there a way to automaticaly send coordinates to the Waze app. from a form?

Unfortunately, JotForm does not have such feature. This seems to be possible with some custom code, here is a link about it: https://stackoverflow.com/questions/44997905/set-custom-coordinates-to-a-function-waze-integration 

But note that developer knowledge is required so you may hire a developer to work on this.
