Can I create a questionnaire with check boxes in two columns?

Hello, I wish to create a questionnaire that has questions in two columns each question having its own checkbox the is checked if applicable to the client.

I wish to email this to clients as a PDFor embed it into a web form and have the results sent to me via email. At some stage, I may like to integrate this with Infusionsoft. 

Is this possible?
