What plan do I need for my small landscaping company?

i need a form where my customers are able to fill out there name, address, phone number, short message, and UPLOAD PICTURES AND DOCUMENTS. the upload wouldnt be more then 100mb more or less . 

WOULD BE GREAT IF THEY COULD DO THIS FROM THERE SMART PHONE ON MY MOBILE WEBSITE !
