Integration with Salesforce not working.

I created a simple test form to submit data to SF to create a person account.  I set up the SF integration and it looks all fine however I dont see any of the 3 submissions I completed on jotform in Salesforce. Is there a way to view successful submissions through the integrations? or is there an error log if it did not submit to SF?

When you integrate and make a submission, the data should be passed to Salesforce without issues. Is this the form in question? https://www.jotform.us/form/90425578415158
