How can I create a tournament entry form with credit card payment?

    I started creating a tournament entry form on JotForm for our tournaments that we are holding this year.  We would like to be able to use the same form for each individual tournament and request payment for each specific event.  Can I do that on one form or do I have to create a form for each tournament because it depends on how many tournaments that individual will be participating in?

     

    FYI...I have not posted this form yet on my website c/o Weebly.

    You can clone the form you have before if you want to create a new entry tournament form.
