I need help getting a Word document into a form

Could you explain how to get a word document into a form?   I know we use the widget pdf embedder, but it does not want to upload file or says the file not supported.  

Im not familiar on how to change  the word document to a .pdf or html to have it come over to form.

You should be able to export the Word Doc to PDF. If you are using MS Office application, follow this guide here https://support.office.com/en-us/article/save-or-convert-to-pdf-or-xps-d85416c5-7d77-4fd6-a216-6f4bf7c7c110.

If not, go to online services like https://smallpdf.com/word-to-pdf to convert.
